Physicals for Student-Athletes 2024-25
Students in grades 6-12 planning to participate in athletics during the 2024-25 school year must have a physical examination before beginning practice. Local physicians have the physical forms and will conduct your examination in their office. Physical forms are also available at the GRHS and LMS School Building Main Offices as well as the District Activities Office. Call your doctor and make an appointment. A physical examination anytime after May 1, 2024, will be good for all activities during the 2024-25 school year. You may also download and print all required Forms and Physical Forms from this webpage.
